The U.S. Chemical Safety Board had a lengthy meeting in Washington, D.C., Thursday to discuss the investigation into a fatal 2011 fireworks explosion in Waikele that killed five federal subcontractor workers. 

The board unanimously adopted new recommendations for safely disposing fireworks, including vetting subcontractors’ experience and stricter oversight during such disposal jobs. Federal agencies have 180 days to decide whether to adopt the recommendations.
